There are many kinds of evolution items in the Pokémon games. The games often lock off certain evolved forms behind making the previous entry hold a certain item. You will need to do a lot of work for the full Sinnoh PokéDex. Which you actually need to do for this item anyway. You don’t need to capture them all, but you need to at least encounter every one of the Regional Dex Pokémon. You need to head to Pal Park after completing the regional Dex, Professor Oak will then give you the National Dex there.
After you have beaten the Elite Four, you need to do some backtracking. Then, head to Eterna City to get the Up-Grade from Oak. You need to head to Eterna City, then head south from the Pokémon Center and enter the building with the yellow roof before the bridge.
If you don’t have the Porygon yet, are you haven’t gotten to this point in the game, don’t worry. You can always store the candidate for evolution until later. Pokémon Home is also useful for the long-term storage of monsters you’re waiting to evolve.
