To get this item, you need to find a certain merchant. Patches is the merchant you’re after. This particular NPC is found in Murkwater Cave. In the ravine between western and eastern Limegrave, you just jump down using your mount. There you will find a small cave along the wall on the west side. That small cave contains the path to Patches. Fight your way past the undead found in the early section, then take the path to your left. Inside this room will be a chest, along with Patches.
Once you loot the chest, Patches rushes in to attack. The key here is not to kill Patches. If you get the boss down to half its health bar, you can simply wait him out. If you stop attacking him at this point, he will then ask to surrender. You can then leave the dungeon peacefully. Once you reload the zone by fast-traveling back and forth from another area back to Limgrave, return to this cave. Inside you will find that Patches has set up his own shop.
