This item is dropped by the Godskin Apostle & Godskin Noble. You will have to fight both of these bosses back-to-back, so bring plenty of healing items. There’s also a Spiritcaller Snail in the room with the bosses, so it can be a tough fight.

To get to the spot you need to be pretty far into the game. You need to get to the Mountaintops of the Giants, which comes after you enter the Capital City in the Altus Plateau and defeat Morgott. Get the key for the Grand Lift of Rold and head out of the Capital. From there, take the lift to the northeast to reach the Mountaintops of the Giants.

Once you get here, head towards the northeast, along an icy river. This will lead you to a frozen lake on the edge of the zone that’s located west of the Minor Erdtree in Mountaintops East. You need to look for the entrance to the Spiritcaller’s Cave. You will find this in the rock wall by the lake. Look for the fog door and you will have found it. You will need two Swordstone Keys to open it. Traverse the fog and head inside.

Being a Talisman, the effect cannot be improved. You will only be able to leech the same small amount of health with the Talisman regardless of your level. The healing factor is scaled on 3% of total max HP, plus a flat 30. So at the current in-game max of 1,900 HP, you will get 87 HP per hit. The effect will trigger for hits three to seven in any combo, although you don’t have to hit the same target for it to happen. Also, only melee hits count, you cannot use spells to trigger the effect.

If you’re playing and enjoying the game, good for you! You might have questions though, so we have some guides to help you out. When making a character, picking the right class is important.
