Once you have it, give it to a Pokemon and it will heal them in battle. The heal is a random amount each time that Pokemon deals damage in battle. The effect randomizes a percentage of their overall HP, and heals that, usually, it will amount to a heal less than a Potion, so less than 20 HP at low levels.

So what do you do with this item? The most common use of the item is for XP share training. What you do is put a high-level Pokemon in the first slot of a party, with multiple lower-level monsters behind it. Then, go to an area that has Pokemon of a median level where you can reliably one-shot them; they will earn decent XP. This will allow you to safely farm XP for weaker Pokemon, while the Shell Bell heals your main fighter.

The Shell Bell in Scarlet and Violet is also great in Tera Raids, as the healing factor can be clutch in taking on a boss.
